Primary Care Physician (PCP)

Key Responsibilities

• Accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in a panel of up to 450 patients.
• Act as a health coach by helping patients set short- and long-term health goals, partner with them to work toward those goals, and frequently follow up.
• Simplify and prioritize treatment plans so behavior change is actionable for patients and for leading care teams.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES

The PCP independently provides care for patients with acute and chronic illnesses encountered in older adults. The PCP takes full accountability for patient care and outcomes, and seeks consultation from specialists when needed while remaining involved in detailed care. The PCP engages with hospitalists when patients are hospitalized, collaborates with care teams for patients unable to come to the office, and assists in recruitment of patients and additional providers. The PCP leads a care team consisting of a medical assistant, care facilitator, and care coordinator, and helps cover for other providers as needed.

KNOWLEDGE, SKILLS & ABILITIES

Competencies for Success: Availability and Accessibility, Service Orientation, Evidence-Based Medicine, physician leadership, quality improvement, influence, self-care, proficiency in Microsoft Office Suite. Ability and willingness to travel locally and regionally as needed. Fluency in English (verbal and written) and other languages as required.

EDUCATION AND EXPERIENCE CRITERIA

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty. Must obtain or hold an active State Medical License. Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar preferred; board eligibility required. Must have a current DEA number, BLS certification within first 90 days.

PAY RANGE

$214,700 – $306,714 salary.
