Home Health Licensed Practical Nurse LPN Full Time

Position Overview

The Licensed Practical Nurse is an essential part of the team responsible for providing and documenting skilled nursing care in accordance with the developed care plan and physicians\u2019 orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.

Territory Madison and Hunstville Counties

Schedule is Monday - Friday full-time visits\u00a0

Essential Job Functions
\u00a0\u2022 Delivery quality patient care in compliance with physician orders under direction of their Registered Nurse teammate.
\u2022 Perform the continuum of hands on client care through documentation and timely communication with the client's physician and other caregivers
\u2022 Recognizes changes in patient needs and responses requiring intervention and implements care to prevent risk or reduce risk.
\u2022 Educate clients and their family members based on client\u2019s specific needs.
\u2022\u00a0 Provide care in patients home using a variety of skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, PICC line dressing changes etc.

Requirements:
\u2022 An active LPN License in the state of application

\u2022 Valid CPR

Preferred:
\u2022 Medicare Skilled Nursing experience
\u2022 Basic understanding of Oasis
\u2022 1-year LPN experience in a home health or health care setting
\u2022 Home Care Home Base exp a plus
