Evening Shift LPN
Job Summary
We are seeking a compassionate and skilled Day & Evening Shift Licensed Practical Nurse (LPN) to join our team at Sapphire Rehabilitation & Care Center in Columbus, Ohio, USA. As an Day & Evening Shift LPN, you will play a vital role in providing high-quality patient care to our residents, while also collaborating with our interdisciplinary team to ensure seamless transitions and exceptional outcomes. Responsibilities
Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Conduct thorough assessments, identify areas of risk, and develop interventions to prevent complications
Document patient information accurately and thoroughly in accordance with facility policies
Maintain open communication with patients, families, and colleagues to ensure continuity of care
Participate in quality improvement initiatives to continuously improve patient outcomes and satisfaction
Demonstrate a commitment to patient-centered care, adhering to our facility's mission.

Requirements
Current LPN licensure in the state of Ohio
High school diploma or equivalent required; associate's degree in nursing preferred
