Personal Supports 1099 contract position

Direct Support Provider for weekends

Make 40 hours from 3pm on Friday to 8am on Monday

every other week-end

Executive Director or designee

Position Summary

The Personal Supports Provider delivers individualized, person-centered assistance to adults with developmental disabilities in accordance with the Florida Medicaid Waiver Handbook and organizational policies. This role focuses on helping individuals develop and maintain skills that increase independence, community inclusion, personal safety, and overall quality of life. Services may occur in the individual’s home or in community settings.

Required

• High school diploma or equivalent.
• At least 18 years of age.
• Valid Florida driver’s license and reliable transportation with required insurance to be able to transport individuals, as needed.
• Level 2 background screening clearance.
• Completion of all APD-required training prior to providing services.
• Experience working with individuals with developmental disabilities.

• Strong communication, problem-solving, and organizational skills.

Essential Duties and Responsibilities

1. Direct Support & Skill Building

• Provide training and assistance in daily living skills, including personal hygiene, grooming, dressing, meal preparation, household tasks, and money management.
• Support individuals in developing social, communication, and self-advocacy skills.
• Assist with community participation activities such as shopping, recreation, transportation, and accessing public resources.
• Promote independence by teaching, modeling, and reinforcing skill acquisition rather than performing tasks for the individual whenever possible.
• Follow all support plans, behavior plans, and health-related protocols as outlined in the Person-Centered Support Plan (PCSP).

2. Health, Safety, and Welfare

• Ensure the individual’s environment is safe, clean, and supportive.
• Monitor for changes in health, behavior, or functioning and report concerns immediately.
• Follow all emergency procedures, incident reporting requirements, and risk-prevention strategies.
• Provide supervision as required by the support plan and Medicaid waiver guidelines.

3. Documentation & Compliance

• Complete accurate, timely daily service notes that meet Medicaid waiver documentation standards.
• Maintain required logs, attendance records, and progress summaries.
• Documentation in the system by Friday of every week.
• Protect confidentiality and comply with HIPAA and organizational privacy policies.
• Adhere to all APD, Medicaid Waiver Handbook, and agency policies regarding service delivery, billing, and professional conduct.

4. Professionalism & Collaboration

• Maintain positive, respectful communication with individuals, families, guardians, support coordinators, and agency staff.
• Participate in required training courses, including CPR/First Aid, Zero Tolerance, HIPAA, and all APD-mandated courses.
• Attend team meetings and contribute to ongoing service planning and quality improvement efforts.
• Represent the organization with professionalism, integrity, and a commitment to person-centered practices.
• Ability to safely support individuals in community settings.

Work Environment

• Services may be provided in the individual’s home, community locations, or other approved settings.
• Work hours vary based on the individual’s support plan and may include evenings or weekends.
