Nurse Practitioner The Nurse Practitioner applies advanced education and clinical competencies to achieve patient outcomes. The Nurse Practitioner works on problems of diverse scope and complexity ranging from moderate to substantial. The Integrated Home Care Program is a creativeinitiative dedicated to transforming healthcare delivery for high-risk patients in the comfort of their own homes. We aim to reduce avoidable hospitalizations and readmissions by providing comprehensive, patient-centered care to individuals with medical complexity and functional limitations. Our team delivers proactive, coordinated care directly where our patients need it most. Main Responsibilities: Conduct comprehensive in-home evaluations and examinations Use both in-person and telemedicine capabilities to deliver care Develop and implement personalized treatment plans Manage acute clinical issues in the home Maintain accurate documentation Participate in medical team meetings You will report to our Chief Medical Officer for San Antonio, Texas. As part of this role, you will work onsite visiting patient's homes in San Antonio, Texas. If selected for this role, we will require you to undergo a TB screening test. Required qualifications: Nurse Practitioner or Physician Assistant with 1+ years of experience in Home Health, or a related field. Preferred qualifications: Bilingual in Spanish and English Additional Information: Language Proficiency Testing: any associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government. Scheduled Weekly Hours: 40 Pay Range: $123,800 - $170,300 per year This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
