HOW YOU'LL MAKE A DIFFERENCE: As a Registered Nurse Case Manager (RNCM) you will make a difference in the lives of our patients. As a RNCM you will provide one‑on‑one patient care, work at the top of your license, and utilize your leadership skills in coordinating care while delivering home‑based nursing care directed by an attending physician. You will be part of an interdisciplinary team focused on compassionate, quality care and positive outcomes, interacting with patients’ families and sharing in every step of their recovery journey. HOW YOU'LL WORK: You will assume ultimate accountability and leadership for the assessment of patient condition and the plan of care. You will case‑manage and provide clinical care under physician direction, educate patients and families, ensure patient safety, manage your time autonomously, and perform all duties within your scope of practice. MAJOR AREAS OF RESPONSIBILITY: Observe and monitor patient conditions and perform OASIS assessments to develop individualized care plans and adjust as needed. Administer medications as prescribed and prioritize visits for high‑risk patients to reduce readmissions. Oversee and supervise the total care of patients provided by nurse aides and LPNs. Manage multidisciplinary care, promote continuity of care, and coordinate admissions, transfers, and discharges. Educate patients and families on disease processes using teach‑back methods to ensure understanding. Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaborate with interdisciplinary teams and physicians to ensure optimal care. Understand and follow agency policies, procedures, rules, and regulations; communicate schedule or availability changes to schedulers or supervisors. Attend in‑service training and mandatory agency meetings. REQUIREMENTS: Registered Nurse with current license in the state of employment. Minimum one year of experience as an RN in an acute care setting. Home care experience preferred. Valid driver’s license and auto insurance in your name as a driver.
