The Client Care Manager plays a key role in delivering this mission by guiding families through care decisions, building meaningful relationships, and ensuring every client receives safe, high-quality, and personalized care.

Position Summary

The Client Care Manager serves as a trusted guide for families exploring home care services and is responsible for ensuring the highest quality of care is delivered in the home. This role focuses on understanding each client's unique needs, developing personalized care plans, supporting Care Professionals, and maintaining strong relationships with clients, families, and community partners.

Responsibilities

• Play an integral role in helping Right at Home improve the lives of individuals and families throughout our community.
• Answer incoming calls with empathy and the intent to improve the quality of life for every caller.
• Listen carefully and ask thoughtful, open-ended questions to understand each family's needs and concerns.
• Provide education on home care services and share helpful community resources when appropriate.
• Schedule and conduct in-home assessments and coordinate care start dates.
• Develop, implement, and maintain individualized care plans that create extraordinary client experiences.
• Maintain ongoing communication with clients, families, Care Professionals, and referral partners.
• Conduct regular announced and unannounced supervisory visits to ensure safe, meaningful, and high-quality care.
• Address client concerns promptly and communicate Care Professional feedback to the recruiting team.
• Facilitate Care Professional and client introductions and provide training support when needed.
• Provide direct client care when necessary to support continuity of services.
• Actively seek client feedback and encourage reviews to help share positive experiences within the community.
• Build and maintain strong relationships with hospitals, clinics, assisted living communities, skilled nursing facilities, social workers, case managers, and other healthcare professionals.
• Conduct random drug testing during supervisory visits as required.
• Assist with answering phones and scheduling shifts as needed to support office operations.

Education / Skills / Abilities / Availability

• High school diploma required; Associate degree in social work, healthcare, or public service preferred.
• Active CNA license in Nebraska required.
• Strong organizational and computer skills.
• Excellent communication and interpersonal skills.
• Strong telephone and customer service skills.
• Knowledge of common medical terminology preferred.
• Bilingual in English & Spanish a plus
