**Join Our Team as a Dynamic Case Manager!** **Job Description:** - **Position:** Case Manager - **Profession:** Registered Nurse - **Specialty:** Case Management - **Shift:** Day 5x8-Hour (08:00 - 16:30) - **Scheduling Type:** Other - **Patient Ratios:** Days: 1:20 - **Float Required:** Yes, based on department needs - **Weekend Coverage:** Required every other weekend - **Pre-Approved Time Off:** 2 Requests - **Patient Age Groups:** Adolescents, Adults, Pediatrics, Geriatrics - **Daily Census:** Case load up to 25 **Experience Required:** - **Years of Experience:** 2 years (acute hospital case management experience performing care coordination and discharge planning) - **Travel Experience Required:** Yes (First-time travelers will be screened with Clinical’s approval) **Skills and Knowledge:** - Care coordination - Discharge planning - Needs assessment / Order DME - Admission criteria - Appeals and denials - Benefits eligibility - Prior authorizations (preferred) - Regulatory knowledge (CMS and HIPAA guidelines) **Additional Skills:** - Experience with HMOs and IPAs - Understanding of advanced directives, POLST, and protective services processes - Familiarity with DRG (Diagnosis Related Groups) and workers compensation (preferred) **Support and Resources:** - **Interdisciplinary Support:** Social Services - **Special Procedures/Unit Details:** Must have current acute care hospital case management experience - **Housing Assistance:** Resources available for finding housing, including potential realtor support. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
