Title: Nurse Practitioner, Advanced Practice Provider Company: Oak Street Health Location: 905 State St, Cayce, SC 29033 Salary: $132,825 - $155,925 ( wider range available based on experience and location) Role Description The purpose of a Nurse Practitioner at OakStreetHealth is to provide effective and equitable value-based primary care to adults on Medicare, keeping them healthy and out of the hospital. NP’s see patients independently and work collaboratively with physicians. They offer longitudinal care, build meaningful patient relationships, and aid patients in understanding their care plans. Practitioners work in outpatient practices with a collaborative care team that includes physicians, NP/PA, medical assistants, scribes, registered nurses, and care managers. Core Responsibilities Provide exceptional primary care and conduct office visits for routine and acute issues. Administer annual wellness visits and health risk assessments with a holistic view of health. Coordinate care with other providers, specialists, testing facilities, and agencies. Lead population health initiatives, such as ensuring eligible females receive evidence-based breast‑cancer screening every two years. Assist the care team with phone triage and outreach. Educate patients on health conditions, care plans, and treatments. Participate in Oak Street Health promotional activities. Conduct home visits as needed. Perform other duties as assigned. Required Qualifications Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate. National certification in one of the following specialties: Family NP, Adult‑Gerontology PC‑NP, Adult‑NP, Gerontological NP. Active, non‑probationary state NP license. Active DEA license. U.S. work authorization. Preferred Qualifications Experience in primary care (internal medicine, geriatrics, or family medicine). Knowledge of HCC (Hierarchical Condition Categories), ICD‑10 coding, and HRAs. Team‑oriented mindset and willingness to collaborate across functions. Desire to be part of an innovative, empirically guided population health model. Bilingual proficiency in applicable areas. Pay Range The typical pay range: $78,331 – $168,714. Actual base salary will vary by experience, education, geography, and other factors.
