Responsibilities Assess and address the social, emotional, and psychological needs of residents and their families Develop and implement individualized care plans to promote resident well-being and independence Provide counseling and support to residents and their families to address issues related to end-of-life care, grief, and loss Collaborate with the interdisciplinary team to ensure comprehensive care and support Coordinate referrals to community resources and services to meet the needs of residents and their families Maintain accurate and detailed records of resident care and services provided Requirements Bachelor's degree in social work or human services Strong communication, interpersonal, and problem-solving skills Ability to work effectively with residents, families, and interdisciplinary team members Familiarity with Medicare and Medicaid regulations and guidelines Current licensure/certification as a Social Worker in the state of Michigan Benefits Competitive pay Double Pay on Holidays Referral Bonuses Life Insurance 401K Health insurance Tuition Reimbursement Employment Briarwood Nursing and Rehab is an equal opportunity employer.
