Physical Therapist

Provides evaluation and therapy services including direct patient care, patient/family education and transition of care. Implements systematic patient examination and evaluation using comprehensive compliant and safe clinical care demonstrating the philosophy of family centered care. Works with medical staff and multidisciplinary team to design and implement individualized plans of care. Creates appropriate discharge plans by collaborating with patients, families and the interdisciplinary team. Functions as a team member and develops leadership skills through mentoring and providing education to students and other professionals. Supports risk management and compliance initiatives through accurate, thorough documentation. Requirements Bachelor's degree in Physical Therapy PT licensed in state of practice Basic Life Support (BLS) certification 1 year physical therapy experience; new graduates considered Pediatric experience #J-18808-Ljbffr
