Nurse Practitioner

Position Overview Reliant Medical Group, part of the Optum family of businesses, is seeking a PCP-Internal Medicine APC (NP or PA) to join our team in Milford, MA. As a member of the Optum Care Delivery team, you will play an integral role in our vision to make healthcare better for everyone. Primary Responsibilities Independently manage a defined panel of primary care patients, providing longitudinal, relationship-based care. Perform comprehensive assessments, diagnoses, and treatment of patients in an ambulatory primary care setting. Manage acute and chronic conditions and serve as the author and supervisor of patient care plans. Make clinically appropriate determinations regarding in‑office management versus referral to specialty or emergency care. Manage inbox responsibilities, including test results, patient messaging, medication management, and referrals. Coordinate care with interdisciplinary team members including RNs, pharmacists, population health, behavioral health, and specialists. Partner with patients and families to support shared decision‑making and whole‑person care. Provide consultation regarding hospitalized patients within the assigned panel. Support practice efficiency, quality initiatives, and standardized clinical workflows. Participate in a shared, site‑based call structure aligned with team‑based primary care. Spend less time charting with DAX AI ambient scribe technology. Practice efficiently with SmartRx and robust EPIC tools, including 30+ medication templates. Qualifications – External Required Qualifications Board Certified as a Nurse Practitioner (NP) or Physician Assistant (PA). Must have at least 3 years of paneled APC experience. Unlimited licensure in the state of Massachusetts. Active DEA license. Preferred Qualifications Experience working within a Team Care model. Fluency in EMR systems & technology. EPIC experience a plus. Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonuses and is based on several factors including local labor markets, education, and work experience.
