Position Overview

We are seeking a skilled RN for a Home Health role in the Roseburg, OR area. This position will provide essential care to patients in their homes and participate in regular office meetings.

Contract Details

• Contract Duration: 13 weeks
• Shift Schedule: Monday to Friday, 8:00 AM - 5:00 PM
• Weekend Rotation: No
• On-Call Requirement: Potential for rotating on-call duties with 4 RNs

Responsibilities

• Conduct home visits for patient assessments and care.
• Attend periodic office meetings.
• Manage a caseload of 5 to 8 patients daily.
• Document patient visits and related activities within designated timelines.
• Complete OASIS assessments and other necessary documentation.
• Travel to patient homes within the Roseburg and surrounding areas.
• Maintain consistent communication with the care team and report any patient needs.

Travel and Productivity Expectations

• Weekly Travel: Approximately 100 miles
• Weekly Productivity Expectation: 30 points, equating to a typical workload of 5 to 10 patients per day.
• Point Values: Different activities such as travel, documentation, and visit types hold varying point values.
• Travel Reimbursement: Miles traveled between patient homes are reimbursable at the IRS standard rate, with certain conditions on billing for travel to the first and from the last patient being non-reimbursable.

Qualifications

• Minimum of one year of recent adult home health or hospice experience.
• Prior experience with Homecare Homebase (HCHB) or similar EMR systems required within the last six months.
• Current CPR and BLS certifications.
• Active nursing license required; pending licenses will not be accepted.
• Ability to travel up to 75 miles from the assigned care center.
• Valid driver's license, reliable transportation, and agency-required liability insurance are mandatory.

Additional Information

This role does not guarantee a fixed number of hours; it is essential for candidates to be flexible and willing to adapt to varying patient loads. If patient census is low, the RN may have opportunities to float to nearby clinics for additional hours.
