Ambulatory Care Nurse - Boynton Beach, Florida

Position Summary We have an exciting opportunity to join our team as an Ambulatory Care Nurse. The Ambulatory Care Nurse will support the coordination, integration, communication, implementation and evaluation of nursing practice and clinical policies and procedures. In this role, the nurse participates in site and organizational quality and performance improvement activities, collects patient healthcare data systematically, and collaborates with licensed providers to develop patient‑centered plans of care that prescribe interventions to attain expected outcomes. The role provides comprehensive care in support of all patients, including administration of treatments and medication, disease prevention guidance, rehabilitation care, public health care, and supportive care for symptom relief including health teaching and counseling. Responsibilities Perform physical examinations and diagnostic tests Collect patient health history Record information in electronic health record Provide information, counsel, and education to patients and families regarding test results and treatment plans as directed Administer medications (including vaccination, IV therapy, oxygen administration), provide comprehensive wound care, and perform phone and in‑person patient triage for clinical concerns Assess and monitor patient responses to treatments, procedures, and anesthesia Consult with clinical supervisors and licensed providers to determine best treatment plans for patients Direct, supervise, and delegate to licensed practical nurses, medical assistants, and technicians as needed to support patient flow and efficiency Perform other duties as needed Clinical Responsibilities Support the coordination, integration, communication, implementation and performance evaluation of nursing practice and clinical policies Ensure clinical compliance with Patient Safety initiatives and reporting Support key quality initiatives (e.g., MIPS, Care gap management) to impact clinical outcome measures Demonstrate knowledge of the NYULH Service Standards and incorporate into performance of responsibilities Participate in activities related to NYULH clinical standards and relevant outside agency regulations Collaborate with members of the interdisciplinary team and promote collegial relationships Serve as a support and resource for nursing, other clinical staff, and providers Participate in clinical inquiry and Act as a mentor to peers and colleagues Identify and report professional misconduct appropriately Implementation / Provision of Care Execute prespecified medical regimens as directed by licensed providers Assess patients, identify unmet patient needs, and plan comprehensive care Identify unidentified co‑morbidities or emergent complications and report to provider for intervention Integrate care with other health care providers (e.g., respiratory therapists, physical therapists, social workers, physicians, nurse practitioners, dieticians, navigators, discharge planners) Provide direct care services, including administration of treatments and medication, sentinel monitoring, disease prevention guidance, rehabilitation care, public health care, and supportive care for symptom relief Task/Procedure Collect and document patient clinical history and assessment data Perform onsite testing (e.g., POC lab testing, EKG, specimen collection, auditory & visual test, psychiatric & physical risk screening, fetal monitoring, spirometry, allergy skin testing, bladder & TB skin testing) Perform medical and nursing treatments per plan of care, provider orders, and NYSBON scope of practice Perform procedures (e.g., hot & cold applications, removal of orthopedic appliances, casts, splints, sutures, staples, urinary bladder irrigation, catheter care, ear irrigation) Apply and remove dressings and perform wound care Assist with minor and major procedures Perform patient monitoring before, during, and after procedure, testing, medication administration, and sedation Administer medication (IM, PO, SQ, ID, IV topical, inhalation, ocular, vaginal, rectal) and provide oxygen therapy, following nursing rights and policies Facilitate medication refills by reviewing clinical information and preparing order for provider authorization in EMR Administer IV therapy, including blood products and chemotherapy with proper certification Start and discontinue peripheral IV lines and maintain peripheral and central IV lines while managing complications Communication and Message Management Ensure clinical in‑baskets are managed and encounters closed in a timely manner Receive, document, and report critical lab values as indicated Relay normal and abnormal clinical results, follow‑up, and explanation to patients, as directed by provider Perform preliminary review of results for prioritization of urgent findings and notify provider for final verification Perform in‑person and phone triage for patient clinical concerns with appropriate escalation and provide clinical advice within RN scope Perform care/case management of a defined population via multidisciplinary plan to coordinate or facilitate patient care as needed Health Promotion & Education Identify, through case‑finding and critical analysis, patient, family, and community deviations from health and well‑being and recommend appropriate actions Collect patient healthcare data systematically and continuously using appropriate assessment techniques and instruments Analyze assessment data to determine health status and nursing diagnoses Report deviations requiring medical diagnosis or treatment to provider and elevate urgent or emergent deviations to medical provider or emergency care Identify expected outcomes and anticipate unexpected or adverse outcomes individualized to the patient Consider needs and behaviors of specific patient age and cultural groups in all patient care activities Assess patient’s ability to adapt to new situations and circumstances created by changes in health status and/or environment Identify barriers to optimal care (e.g., physical, cognitive, language, literacy, socioeconomic, cultural) Assess and identify learning needs; document and update accordingly Develop patient‑centered plans of care that prescribe interventions to attain expected outcomes and partner with patients to set individualized goals Educate patients and families to facilitate and promote healthy adjustment to new or changed health patterns Implement interventions identified in the plan of care and evaluate patient progress toward attainment of expected outcomes Support Certification through the American Heart Association Fluent written and spoken English language skills Current Registered Professional Nurse in Florida Required Licenses Registered Nurse
