The role involves assessing and addressing the physical, psychological, social, and spiritual needs of patients and their families, initiating care upon admission, and collaborating with an interdisciplinary team to ensure comprehensive care. Responsibilities: Identify patient and family needs. Initiate appropriate interventions upon admission. Collaborate with the care team for a responsive care plan.
