Job Details Position: Physical Therapist – Home Health (Full-Time). Primary location: Missouri City, Stafford, Pearland, Manvel, Rosharon, TX. Overview TheraCare is seeking a licensed Physical Therapist for a full-time home health opportunity providing in-home physical therapy services to adult and geriatric patients throughout Missouri City and surrounding areas. This role offers flexibility, autonomy, and the ability to provide one-on-one patient care in the home setting, allowing therapists to create their own schedules while improving patients’ mobility, strength, safety, and overall independence. Job Responsibilities Perform home health physical therapy evaluations and re-evaluations, including OASIS assessments, for adult and geriatric patients Develop, implement, and update individualized Plans of Care (POC) based on patient goals, diagnoses, and physician orders Provide skilled physical therapy treatment for orthopedic, neurological, cardiac, post-surgical, and medically complex conditions Deliver evidence-based interventions focused on mobility, gait training, balance, strengthening, fall prevention, pain management, and functional independence Apply appropriate therapeutic techniques and physician-ordered modalities, including manual therapy, heat, cold, and electrical stimulation Educate patients and caregivers on home exercise programs, safety awareness, transfer training, fall prevention, and use of assistive and durable medical equipment Monitor patient progress toward functional goals and modify treatment plans as clinically indicated Complete accurate, timely, and compliant documentation within the EMR at the point of care Communicate and collaborate effectively with physicians, nurses, case managers, PTAs, home health aides, and interdisciplinary care teams Supervise Physical Therapist Assistants (PTAs) and home health aides in accordance with state regulations and agency policies Participate in case conferences, discharge planning, care coordination, and ongoing interdisciplinary communication Ensure compliance with home health regulations, Medicare guidelines, infection control standards, and patient safety protocols Maintain professional competency through ongoing education, training, and adherence to evidence-based practice standards Perform additional duties as assigned to support quality patient care and operational excellence What We Offer Competitive Compensation Package Mileage Reimbursement Over-Productivity Pay Comprehensive Health, Vision, and Dental Plan Options Supplemental Benefits (Life, Disability, and Accident Insurance) 401(k) with Matching Contributions Paid time off (Vacation/Sick/Holiday) for Full-Time Employees CEU Reimbursement Employee Recognition Program with Financial Rewards Patient Population Adults and seniors Post-surgical recovery Stroke and neurological conditions Chronic pain and mobility limitations Fall-risk patients in the home setting Job Details Employment Type: Full-Time Schedule: Flexible daytime schedule Work Setting: Home Health / In-Home Physical Therapy Locations: Missouri City, Stafford, Pearland, Manvel, Rosharon, Fresno, TX Qualifications Doctorate in Physical Therapy from an APTA-accredited program Active Physical Therapist (PT) license in the state of Texas or eligible Compact PT Licensure Home Health experience (1 year required) HomeCareHomeBase (HCHB) experience (preferred) Current CPR certification Strong communication, clinical judgment, and time-management skills Knowledge of OASIS and home health regulations preferred Valid driver's license, reliable transportation, and current auto insurance Equal Opportunity Employer We are an Equal Opportunity Employer. #J-18808-Ljbffr Cantex Continuing Care Network
