This role is responsible for assessing patient needs, coordinating care, administering treatments, providing health education, and supporting providers to improve patient outcomes. Location: Glendive Schedule: Full-time | M/W-Th 7:45 am - 5 pm, Tues 8 am - 5 pm, F- 7:45 am - 1 pm | No Weekends Salary: LPN $27-29/hr DOE Key Responsibilities – What You’ll Do: Deliver Patient-Centered Care – Conduct assessments, provide treatments, and ensure strong follow-up care. Collaborate Across Teams – Partner with providers and healthcare staff to create effective care plans. Educate & Empower – Support patients and families with knowledge on conditions, treatments, and prevention. Administer Medications – Safely deliver medications and treatments per protocols. Maintain Records – Accurately document patient care and changes in condition. Drive Quality Improvement – Participate in initiatives that enhance care and outcomes.
