Clinical Pharmacist – Home Infusion

Schedule: Monday–Friday with rotating shifts:

• 8:00 AM – 5:00 PM
• 8:30 AM – 5:30 PM
• 9:00 AM – 6:00 PM

Additional Requirements: Evening and weekend on-call coverage required. Overtime available as needed.

Required Experience: MUST have Home Infusion experience.

Position Summary

We are seeking an experienced Clinical Pharmacist – Home Infusion to oversee and manage home infusion therapies in a fast-paced, patient-focused environment. This role combines clinical oversight, sterile compounding, and operational workflow management while ensuring safe, compliant, and timely delivery of infusion medications.

The ideal candidate will have strong home infusion experience, expertise in sterile compounding, and the ability to collaborate effectively with providers, nurses, and pharmacy staff to deliver exceptional patient care.

Responsibilities

• Review referrals for appropriateness, including medication, dose, route, laboratory values, and therapy requirements.
• Manage new patient starts, refills, medication reconciliations, and patient counseling.
• Monitor lab-dependent therapies and coordinate care with physicians, nurses, and other healthcare providers.
• Perform accurate prescription entry, documentation, and claim adjudication using CPR+.
• Prepare and verify sterile compounded medications in compliance with USP standards.
• Ensure proper labeling, lot tracking, and quality assurance procedures are followed.
• Supervise pharmacy technicians and support daily pharmacy production and workflow operations.
• Assist with inventory management and ensure adequate pharmaceutical supplies are maintained.
• Coordinate medication dispensing and delivery to ensure safe, accurate, and timely patient care.
• Maintain compliance with all company policies, state and federal regulations, and pharmacy practice standards.

Qualifications

• Doctor of Pharmacy (PharmD) degree required.
• Active Pharmacist license in good standing required.
• Home Infusion experience is required.
• Experience with sterile compounding and USP compliance required.
• Previous experience with CPR+ preferred.
• Strong clinical assessment, communication, and organizational skills.
• Ability to thrive in a fast-paced, collaborative environment and participate in evening and weekend on-call rotations.
