About the Role:

We are seeking an experienced Nurse Practitioner or Physician Assistant with a background in Geriatrics or Internal Medicine to join our team. You will be responsible for:

This is a part-time position of 19 hours per week

• Providing patient-centered care to residents in skilled nursing facilities

• Conducting on-site visits and remote work from home

• Collaborating with our dedicated scribe team for efficient documentation

• Building strong relationships with facility staff and maintaining a patient-centered approach

• Being a collaborative team player and contributing to our positive and supportive clinical environment

Requirements:

• Active NP or PA license (in the state of practice)

• 1+ year of experience in:

  • Geriatric care

  • Documentation review

• Excellent communication, collaboration, and teamwork skills
