Hospice RN Case Manager Where Support Meets Impact
Lower patient-to-nurse ratios for meaningful, unrushed hospice care
Strong interdisciplinary team supportcollaborative care with Social Work, Chaplaincy, Aides, and providers, so you're never practicing alone
Leadership that understands hospice nursing
Dedicated internal after-hours triage team to support nurses outside of regular business hours
Homecare Homebase (HCHB) EMR for streamlined documentation
Office Location: 1028 S. Main Street Suite C Fond du Lac, WI 54935 Coverage area: Travel throughout the territory is required - Serving Winnebago & Fond du Lac Counties
Schedule: Monday-Friday 8AM-5PM

Responsibilities
As a RN Case Manager You will:
Assess/monitor physical, emotional, and psychological needs of patients
Create hospice care plans that align with the patient's wishes and goals
Direct nursing care: administering medications, treatments, and interventions
Provide pain and symptom management
Educate and support the patient's family and caregivers
Collaborate with interdisciplinary team
Maintain accurate and timely documentation
Participate in on-call rotation as required by local branch
Qualifications
Graduate of an accredited nursing school with current licensure to practice in state of operation
One year nursing experience required, 3+ years preferred
Experience in hospice or a similar setting preferred
Knowledge of the hospice philosophy of care
Commitment to clinical and documentation excellence
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
