Clinical Pharmacist Specialist - RWJBarnabas Health

At RWJBarnabas Health, the Clinical Pharmacist Specialist plays a critical role in specialized patient care and operational excellence. Operating with a high level of independence, they ensure that advanced medication therapies are provided in a safe and rational manner according to legal, professional, and institutional standards. They are responsible for delivering expert drug information, monitoring complex prescribing criteria, designing clinical guidelines, analyzing interventions, and serving as a liaison between the Pharmacy Department and the interdisciplinary medical staff across multiple specialized care areas.

A typical day for a Clinical Pharmacist Specialist may include:

• Assumes the role of expert drug information provider, delivering patient-specific and age-appropriate consults to all members of the healthcare team
• Directs clinical coverage and specialized pharmaceutical care across priority areas, including Perioperative services, Transplant, Emergency Medicine, Internal Medicine, Geriatrics, NICU, Antimicrobial Stewardship, and Critical Care (ICU, BURN, CTICU, Step-Down) [1]
• Acts as a key liaison between the Pharmacy Department and medical or nursing staff regarding prescription orders, therapeutic details, patient reactions, and clinical complaints
• Monitors and evaluates complex medication prescribing prior to dispensing, ensuring strict adherence to drug usage criteria established by the Pharmacy & Therapeutics Committee
• Advises on clinical goals, policy and procedure updates, order set and guideline development, departmental communications, and pharmacy financial forecasting
• Documents, analyzes, and provides robust data support for all clinical interventions, medication errors, and adverse drug reactions according to departmental criteria
• Utilizes a highly analytical medication screening process to ensure all pharmacy orders are prioritized, complete, clinically appropriate, and within legal parameters
• Serves as a dedicated preceptor for pharmacy students and pharmacy residents, actively supporting the success of College of Pharmacy partnerships and PGY-1/PGY-2 residency programs
• Enhances professional growth and institutional quality improvement initiatives by participating in educational programs, current literature review, and facility-wide committee projects

To be considered for this Pharmacist I opportunity:

• Graduate of an accredited School of Pharmacy with a Bachelor of Science in Pharmacy or a Doctor of Pharmacy (PharmD) degree
• Must hold and maintain an active Registered Pharmacist license in good standing from the New Jersey Board of Pharmacy
• Successful completion of an ASHP-accredited PGY-1 Pharmacy Residency program or equivalent clinical practice experience is required
• Must obtain a Board of Pharmacy Specialties (BPS) Certification within one year of employment
• Must obtain Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications within six months of employment
• Post-Graduate Year 2 (PGY-2) Pharmacy Residency completion or equivalent specialized clinical experience is highly preferred
• Availability to work on a Per Diem basis, Monday through Friday, covering day shifts as needed to support specialist clinical operations
