Nurses work alongside clinicians, behavioral health, case management, health education, and community services to support whole person care that addresses medical needs and social drivers of health.

Position Summary:  LCPD is seeking a compassionate and collaborative Registered Nurse to join our growing Perinatal Care Team for a full-time position. This is an opportunity to support an outpatient program that provides comprehensive prenatal, obstetric, gynecologic, family planning, and sexual health services across two clinic sites with a multidisciplinary team composed of an Ob/Gyn, a certified nurse midwife, family practice clinicians, and community health workers and educators. The goal of the program is to deliver high-quality, evidence-based and culturally appropriate care while contributing to a culture of continuous improvement, collaboration, and equity. 

The RN responds to provider orders and referrals, conducts nursing assessments, performs telephone and in-person triage, provides patient education, coordinates care, and delivers nursing services within scope of practice. The RN supports patients across the continuum of sexual and reproductive health care, including prenatal care, obstetrics, gynecology, contraception and family planning services, STI prevention and treatment, and sexual health education. The RN also supports transitions of care, population health initiatives, and quality improvement efforts while promoting health equity and addressing social drivers of health.   

Qualifications: Required Experience & Education:

• Currently licensed as a Registered Nurse in Maryland and/or the District of Columbia .  Both licenses are required within 90 days of hire. 
• Bachelor’s degree in nursing
• At least 3 years of experience in obstetrics/prenatal clinic or labor and delivery care, including performing and interpreting non-stress tests and triage of obstetric patients.
• Proficiency in written and spoken English and Spanish
• Current BLS certification.
• Strong clinical assessment and decision-making skills.
• Commitment to health equity and serving vulnerable populations.
• Demonstrated ability to lead and collaborate within interdisciplinary teams.

Preferred:

• Experience working in FQHCs or community health settings
• Experience working with other diverse populations including the Latine immigrant community, LGBT+ community, individuals experiencing homelessness and/or individuals with substance use disorders.

Key Responsibilities

Clinical care & coordination

• Conduct nursing assessments and develop individualized care plans in collaboration with obstetric providers, certified nurse midwives, primary care providers, and other members of the care team.
• Exercise independent clinical judgment in evaluating patient concerns and providing nursing interventions within scope of practice.
• Conduct in-person and telephone triage for prenatal, postpartum, and gynecologic patients, collaborating with providers to ensure timely and appropriate care.
• Perform prenatal intake assessments, nursing visits, blood pressure monitoring, patient screenings, medication administration, and other nursing services according to organizational protocols.
• Perform non-stress tests (NSTs) and other perinatal nursing procedures according to clinical protocols.
• Provide education regarding pregnancy, postpartum recovery, gestational diabetes, hypertension, contraception, mental health, and other relevant health topics in collaboration with the multidisciplinary team. 
• Review laboratory and diagnostic results, coordinate follow-up care, and facilitate timely communication with patients and providers
• Support patients in self-management and behavior change using patient-centered education, coaching, motivational interviewing, and culturally responsive communication.
• Proactively identify opportunities to provide preventive care, health screenings, immunizations, internal referrals, and other nursing interventions within scope of practice.
• Serve as a clinical resource and advocate for patients, ensuring equitable, trauma-informed, and culturally responsive care.
• Collaborate closely with interdisciplinary care team to support whole-person care, address social drivers of health and barriers to care.
• Participate in regular multidisciplinary team meetings and shared care planning.
• Coordinate care across internal and external providers to promote continuity of care, including patient navigation and referral management throughout pregnancy and the postpartum period.
• Support primary care team and perform duties of primary care clinical nurses as needed. 
• Other duties as assigned. 

  Program support

• Participate in population health outreach activities and support efforts to improve prenatal engagement, postpartum follow-up, and quality outcomes.
• Support perinatal and sexual health team in submitting required clinical documentation for state and federal program compliance.
• Participate in clinical quality improvement and risk management activities in collaboration with the perinatal clinical team.

Leadership

• Serve as a clinical resource to primary care nurses, medical assistants and other team members regarding perinatal care workflows and clinical protocols.
• Assist with staff education, competency development, and clinical training activities.
• Support nursing leadership in fostering a respectful, inclusive, and collaborative team culture.
• Champion evidence-based practices and contribute to protocol development and implementation.
• Demonstrate dedication to ongoing learning and development of clinical knowledge and skills via participation in professional development and learning opportunities.
