Registered Nurse - Home Health

Job Description

Job Summary:
Provide comprehensive home health nursing care to patients in their own residences, ensuring adherence to care plans and quality standards. Collaborate with interdisciplinary teams to facilitate patient recovery and well-being.

Responsibilities:

Assess patient's health status and develop individualized care plans.
Administer medications and treatments as prescribed.
Document patient progress and update electronic health records accurately.
Educate patients and families on health management and safety.
Fill in at different locations as needed, including Glendale.
Ensure compliance with all safety and health regulations, including EPIC documentation standards.

Qualifications:

Hold a valid Registered Nurse (RN) license in the state of practice.
Possess current CPR or BLS certification from an approved vendor.
Demonstrate experience with EPIC electronic health record systems.
Maintain a valid driver's license, automobile insurance, and reliable transportation.
Be at least 21 years of age or older.
Ability to work scheduled hours and rotate weekends as part of the 40-hour workweek.
