Your Role in Our Mission As a Tailored Care Manager you will support individuals by evaluating their needs, developing personalized care plans, and coordinating services that address physical and mental health, social determinants of health, and other life challenges. The role involves traveling throughout Clinton, NC and surrounding counties to help close gaps in care and improve lives in the community. Why Join Us? You’ll earn a competitive salary ranging from $53,000 to $56,000 annually, based on experience and qualifications. This full‑time position offers a primarily Monday‑Friday schedule from 9 AM to 5 PM with rotating on‑call responsibilities and a supportive environment that values work‑life balance.
