Community Connections Case Manager

Job Title: Community Connections Case Manager Department: Foster Care Status: Full-Time, exempt Reports To: Supervisor Compensation Starting Pay: $58,870 annually PURPOSE: The Community Connections Case Manager maintains and strengthens family units, by providing intensive in‑home services designed to prevent children from entering the foster care system. This program works with family strengths, monitors the family dynamics, provides linkages to community resources, assists with immediate needs and concerns, as well as general case management services. Emphasis is on child safety and keeping the family together. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Meet with families in their homes at various hours of the day as well as on call duties as scheduled in potentially high‑risk neighborhoods. Facilitate connections to community‑based resources and supportive services to address challenges impacting family stability and well‑being. Serve as a conduit of information between all parties to the case and identified stakeholders, including medical, educational and mental health providers. Conduct visits with children, parents and/or caregivers that include the ongoing assessment of child safety, permanency and well‑being. Develop & implement a plan for delivery of services & supports in collaboration with the family that meets the immediate needs of the children and the family. Perform case management responsibilities in accordance with state and federal laws, Department of Children and Families Administrative Codes and Operating Procedures, funding source policies and internal policies on child abuse, abandonment, and neglect within required timeframes. Willingness to travel as position requires frequent travel; follows agency and state guidelines regarding driving, rules of the road, and safety. EDUCATION and/or EXPERIENCE REQUIRED/PREFERRED Degree/Field Required: Bachelor's degree in Social Work or a related human services field Degree/Field Preferred: N/A (Related fields will be considered.) Experience Required: 0-6 months case management experience preferred but not required (For positions that manage staff, the years specified need to be progressive years of management experience. For all positions, the years required can be interchanged with equivalent experience from an advanced degree related to this job and/or with other relevant experience verified by applicant/incumbent). Licenses/Certifications Required: Child Welfare Employee License and any additional certifications as mandated by the Department of Children and Family Services.

SKILLS REQUIRED

Realizes the prevalence of trauma among persons served by Caritas and the general population. Responds to trauma by actively implementing the guiding principles of trauma informed care into their daily practice. Resists re‑traumatization of persons served and staff providing services, by creating safe, welcoming, and supportive environments.
