Responsibilities
- Provide skilled physical therapy services to residents under supervision of a licensed physical therapist in a long-term care / assisted living setting.
- Evaluate, treat, and re-evaluate residents; implement the plan of care with therapeutic exercises, modalities, and functional activities.
- Document evaluations, treatment plans, progress notes, and discharge summaries in charting per facility and regulatory standards.
- Collaborate with the interdisciplinary team (nursing, OT, speech therapy) to optimize functional outcomes and support discharge planning.
- Educate residents and caregivers on home exercise programs, safety, balance, transfers, and fall prevention.
- Maintain safety, infection control, and equipment standards; comply with regulatory requirements and privacy guidelines.

Requirements
- Licensed or eligible for licensure as a Physical Therapy Assistant in New Hampshire.
- Associate degree in Physical Therapy Assisting.
- Ability to work effectively in a long-term care / assisted living environment.
- Strong communication and teamwork skills; ability to work under supervision and within a multidisciplinary team.
- Willingness to participate in a 13-week travel assignment and adhere to facility policies and schedules.
