• Assist with medication management and monitoring
• Support with mobility and transfers, as needed
• Encourage and promote independence and self-advocacy
• Maintain accurate and thorough records of client care and services provided
• Develop and implement individualized care plans to meet each client's unique needs
• Collaborate with our interdisciplinary team to ensure seamless care coordination

**Requirements:**
• High school diploma or equivalent required; degree in a related field preferred
