Job Title: Registered Nurse Profession: Nursing Specialty: Hospice Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum of one (1) year as a Registered Nurse either in acute care, oncology, long-term care, hospice, or home health. License: Current license to practice nursing in the state of employment. Certifications: BLS (American Heart Association), current automobile insurance, and valid driver's license. Must-Have: Knowledge of terminally ill patients and their families; excellent patient assessment skills; ability to apply knowledge of hospice services; effective documentation and program management skills. Description: The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to the patient’s family. The RN will manage an interdisciplinary group consisting of support staff to enhance the quality of hospice care. The RN will case manage all assigned patient clinical records. The RN will ensure the highest quality of care is provided and documented in accordance with the Plan of Care. The RN will perform initial and ongoing assessments of patients. The RN will develop and implement the Hospice Plan of Care prescribed by the physician in conjunction with the interdisciplinary group. The RN will work closely with other members of the interdisciplinary group to provide all-inclusive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support to patients and families. The RN will report any changes in the patient’s condition to both the Attending Physician and Hospice Physician. The RN will maintain accurate and updated medical records, including all interdisciplinary notes, Plans of Care, initial and ongoing assessments, and any other pertinent information related to patient care. The RN will practice infection control measures and standard precautions. The RN will monitor scheduling of all staff. The RN will participate in the Quality Assessment and Performance Improvement Program. The RN may be requested to participate in on-call schedules. The RN will consistently promote the organization’s core values. The RN is responsible for education of patients, families, or other caregivers regarding needs identified on the Plan of Care. The RN will prepare written instructions for patient care. The RN will make supervisory visits to the patient's residence at least monthly, either when support staff is present to observe and assist or when absent. The RN will assess the competence of support staff in providing care and determine whether goals are being met. The RN will maintain close contact with the patient’s family and/or significant others to provide information, support, and continuity of care. The RN will maintain primary control and professional management of each patient and act as the primary liaison between physicians, patients/families, and the hospice team. This position is safety-sensitive. The RN will attend staff meetings and maintain a high level of knowledge pertaining to the management of pain and symptoms in the dying process. The RN will perform ongoing reviews and follow-up on assigned patient clinical records to ensure quality services are provided, standards of practice are followed, and compliance with policies and regulatory requirements is maintained.
