Job Summary

Medi Link Health Services, LLC is seeking an energetic, organized, detail-oriented, and dependable Home Health Intake & Therapy Coordinator to join our growing home health agency.

This position is responsible for assisting (back-up) with the patient referral and intake process and coordinating therapy services and schedules. The position requires frequent communication with patients and families, referral sources, insurance companies, physicians, nurses, therapists, and other members of the care team.

The ideal candidate is professional, customer-focused, organized, able to manage multiple priorities, and comfortable working in a fast-paced home health environment. This position will also provide general office support and perform other administrative duties as needed to support the daily operations of the agency.

Pay rate will depend on previous experience.

Education & Experience

• High School Diploma or equivalent required; college degree preferred.
• Previous experience in a healthcare organization preferred.
• Home health experience is a plus.
• Knowledge of medical terminology or healthcare terminology preferred.

Duties & Responsibilities

Referral & Intake Coordination

• Receive and process patient referrals received by phone, fax, email, referral sources, and marketing personnel.
• Assist with the overall intake process and ensure referral information is accurately documented.
• Obtain and enter patient demographic and referral information into the EMR.
• Communicate with referral sources, patients, families, insurance companies, and clinical staff as needed.
• Follow up with the Insurance Verification department regarding patient insurance and eligibility information.
• Assist with gathering required documentation for patient admissions.
• Follow up on missing information or documentation needed to process referrals.
• Communicate referral status and other pertinent information to appropriate clinical and office staff.

Therapy Coordination

• Receive and process requests and referrals for physical therapy and other therapy services.
• Distribute therapy referrals for assignment to appropriate therapy professionals.
• Coordinate therapy schedules with therapists, nursing staff, patients, and families.
• Maintain and update the weekly therapy schedule.
• Communicate scheduling changes to patients, families, therapists, and appropriate staff.
• Coordinate therapy services with the nursing supervisor and professional field staff.
• Ensure patients receive ordered therapy services as scheduled.
• Assist with coordinating medical supplies and equipment for patients when needed.
• Follow up to ensure requested services, supplies, and equipment are received.
• Review therapy activity logs and compare them with schedules and clinical records for accuracy.
• Communicate pertinent patient or scheduling information to the appropriate nursing or supervisory staff.

General Office & Communication Support

• Answer incoming telephone calls in a friendly, professional, and courteous manner.
• Route calls and information to the appropriate staff member and take messages when necessary.
• Respond to routine patient, family, referral source, and staff inquiries or direct them to the appropriate person.
• Schedule and coordinate patient and staff visits as directed.
• Scan, fax, email, upload, and attach documents to the appropriate systems and EMR.
• File and maintain documents accurately and according to agency procedures.
• Assist with preparing, organizing, and maintaining office and patient-related materials.
• Assist with communication between office staff and field clinicians.
• Provide backup coverage for referral/intake and therapy coordination responsibilities as needed.
• Maintain a clean, organized, and safe work area.
• Follow agency policies, procedures, HIPAA requirements, and applicable rules and regulations.
• Perform other related duties as requested by the Nursing Supervisor, management, or agency leadership.

• Be flexible and willing to assist with other office responsibilities when needed.
