Registered Nurse (RN) - Outpatient / Patient Aligned Care Team (PACT)

The Registered Nurse (RN) - Outpatient / Patient Aligned Care Team (PACT) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. The person in this role assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. The RN - Outpatient (PACT) is also responsible for the following duties:

• Assisting in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation.
• Demonstrating leadership in delivering and improving holistic, outpatient care through collaborative strategies with others.
• Evaluating practices in an ongoing process, based on best evidence.
• Providing peers with informal constructive feedback for improvement.
• Fostering a safe and supportive environment conducive to the professional development of healthcare professionals.
• Contributing professional nursing perspective in discussions with the interdisciplinary team.
• Partnering with others to effect change and produce optimal outcomes.
• Supporting colleagues through knowledge sharing to provide safe, quality outpatient nursing care.
• Participating in activities and strategies to sustain evidence-based-practice.
• Other duties, as assigned.

Pay: Competitive salary, regular salary increases, potential for performance awards
