TN Quality Care Hospice is seeking a licensed Social Worker to join their growing team in Camden! This role provides counseling, resources, and coordination with our clinical team to ensure holistic care for both patients and families in the comfort of their homes. Schedule Monday-Friday 8:30am-4:30pm Territory Camden and surrounding counties. Current State Licensure in Social Work – Tennessee: LMSW or LCSW required. Minimum of one year’s experience in health care or hospice setting. Valid driver’s license and proof of insurance required. What You’ll Do Perform the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensure that identified needs are addressed in the patient’s plan of care. Provide an assessment in the patient’s identified residence and assistance if this is not safe, and another plan is required. Demonstrate exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to consistently provide this level of service. Counsel patient and family/caregivers and staff as needed in regard to stress and other identified coping difficulties. Provide accurate and timely documentation of patient services to reflect the plan of care. Provide assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtain resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs.
