Title
RN - Outpatient Case Manager

Schedule
40 hours per week, 3-12 hour shifts with weekend rotation.

Assignment Details
Contract Length: 4 weeks
Guaranteed Hours: 36

Requirements
- Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program
- State licensure as a Registered Nurse (RN)
- Minimum of 3 years RN Case Management experience required
- Experience with Epic EMR

Roles & Responsibilities
- Coordinate the overall admission process for inpatients and outpatients.
- Ensure access to healthcare resources and services.
- Work collaboratively with a multidisciplinary team to optimize patient care.
- Facilitate communication between patients, families, and healthcare providers.
- Identify and resolve barriers to care for patients.
- Monitor and evaluate patient progress and outcomes.

Compliance / Notes
- Travel workers must have a permanent address at least 75 miles away from the facility.
- Orientation and registration requirements apply, including potential background checks.
