Registered Nurse (RN) Case Manager Per DiemField-Based | Concierge Home CareAffirmed Home Care, New Jersey's premier private-pay home care agency, is seeking an experienced and compassionate Per Diem Registered Nurse (RN) Case Manager to provide exceptional in-home clinical oversight throughout Bergen, Essex, Morris, and Passaic Counties. This field-based position is ideal for an independent, organized RN who enjoys building meaningful relationships with clients while delivering high-quality, personalized care in the comfort of their homes.ResponsibilitiesConduct comprehensive in-home nursing assessments.Develop, implement, and oversee individualized plans of care.Obtain and maintain physician orders in compliance with New Jersey regulations.Monitor client conditions and promptly communicate significant changes.Complete accurate and timely clinical documentation.Perform aide supervisory visits and ensure quality of care.Provide clinical guidance and support to Home Health Aides and Nurses.Promote client safety, independence, comfort, and overall well-being.QualificationsActive New Jersey Registered Nurse (RN) License.Minimum of 2 years of recent RN experience in acute care, home care, or case management.Recent experience with: Ventilator and tracheostomy careG-tube/PEG tube managementIV infusionsWound careCurrent BLS Certification.Current physical examination (within the past year).Current PPD or QuantiFERON test (within the past year), or chest X-ray completed within the past five years, if applicable.MMR immunization documentation.Valid driver's license and reliable transportation.Authorized to work in the United States.CompensationRN Assessment: $150 per visitRN Aide Supervision: $125 per visitHourly Clinical Visits: $55 per hourWeekday On-Call: $50 per day Monday–Friday | 5:00 PM – 9:00 AMWeekend On-Call: $110 per day Saturday 8:00 AM – Monday 9:00 AMBenefitsCompetitive compensationFlexible per diem schedulingWeekly direct depositEmployee referral bonus programStreamlined onboarding processSupportive clinical and administrative teamOpportunity to provide one-on-one concierge home careWhy Affirmed Home Care?
