Overview Home Health Registered Nurse (RN) | Flexible Schedule | Full-Time, Part-Time, PRN | Cleveland Area BrightStar Care is seeking an experienced Registered Nurse (RN) to serve as a key clinical team member delivering high-quality care throughout Cuyahoga, Lorain, and Medina counties. This role supports new client admissions, urgent staffing needs, skilled nursing visits, and ongoing case management. Compensation & Benefits Competitive pay rates Premium pay for select short-notice and 2-hour visits Weekly pay with direct deposit Car allowance Phone allowance Medical, Dental, and Vision insurance Supplemental insurance options Paid Time Off (PTO) Paid bereavement leave Continuing education and professional development opportunities Family membership to Middleburg Heights Recreation Center Employee discounts and recognition programs Schedule Full-time employees average 40 hours per week Monday through Friday and every other weekend availability preferred Flexible scheduling based on patient needs Overtime opportunities available Should be available to work in west part of Cleveland Job Responsibilities Skilled Nursing Care Perform initial assessments, reassessments, and safety evaluations Deliver skilled nursing services including wound care, injections, infusions, and other treatments within RN scope of practice Monitor patient conditions and communicate changes appropriately Maintain accurate and timely clinical documentation Follow infection control and safety protocols Case Management & Care Coordination Develop and maintain individualized Plans of Care Coordinate care with physicians, families, and interdisciplinary teams Support client admissions and transitions of care Ensure compliance with agency, state, federal, and Joint Commission standards Staff Support & Supervision Conduct supervisory visits and competency assessments Provide clinical guidance and education to caregivers and nursing staff Assist with orientation, skills validation, and ongoing training Support quality assurance and chart review activities Qualifications Required Current unrestricted Ohio Registered Nurse (RN) license Graduate of an accredited nursing program Minimum three (3) years of clinical nursing experience Minimum one (1) year of case management experience Current CPR certification Valid driver\'s license, reliable transportation, and auto insurance Ability to travel throughout assigned service areas Ability to pass background check, drug screening, and other pre-employment requirements Preferred Home health experience Healthcare management experience Pediatric nursing experience Experience supervising clinical staff Physical Requirements Ability to stand, walk, bend, kneel, reach, push, and pull as required Ability to lift up to 50 pounds with or without accommodation Why BrightStar Care BrightStar Care is a nationally recognized leader in home care and medical staffing.
