Home Health Registered Nurse PRN

Job Overview As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you will manage care plans that support recovery and help patients get back to the life they love. Responsibilities Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments. Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals. Monitor patients' conditions and report changes. Educate patients and their families on disease management, medication, and treatment options. Maintain accurate records of patient care and coordinate with other healthcare professionals. Report patient care and condition progress to the physician and Clinical Manager. Oversee the work of Home Health Aides and monitor their documentation completion. Required Qualifications Med Surg, ICU, ER, or acute experience Current and unrestricted Registered Nurse licensure Current CPR certification Experience collaborating with a team of healthcare professionals Valid driver’s license, auto insurance, and reliable transportation Preferred Qualifications One year nursing experience Home Health experience Compensation

• 54.00 – $75.00 pay per visit/unit
• 84,900 – $116,800 per year base pay

• This role is patient‑facing and is part of Humana’s Tuberculosis (TB) screening program; selection requires TB screening.
• The role requires a valid state driver’s license and personal vehicle liability insurance per state minimum limits (e.g., $25,000 bodily injury per person, $25,000 bodily injury per event, $10,000 property damage, or higher).

Schedule Scheduled weekly hours: 1 (full‑time basis of 40 hours). Compensation may vary based on location and demonstrated skills.
