Care Coordination Administrative Specialist

Job Title: Care Coordination Administrative Specialist Location: Remote Job Type: Full-time, Non-exempt Compensation: $18.00 - $22.00 About Us: Third Eye Health is dedicated to delivering high-quality, nurse-centric support for patient-centered care through innovative telehealth solutions within skilled nursing facilities, provider groups, health systems, and health plans. Third Eye Health delivers seamless, clinician-led patient care during daytime and after-hours coverage periods. Job Summary: The Care Coordination Administrative Specialist supports the Care Coordination Management (CCM) team by helping maintain continuity of care between skilled nursing facilities and Third Eye Health clinicians and by supporting daily CCM and QACS operations. This role combines client and clinician-facing communication with behind-the-scenes administrative support and reports to the Director of Care Coordination. Key Responsibilities: Assist with incoming emails to ensure timely processing of CCM workflow tasks from Third Eye Health Dashboard. Follow-up directly with client facility or review the Electronic Health Record (EHR) for patient follow-up information and complete an addendum to the EHR note. Communicate with clinicians and providers via email regarding annotations in the EHR. Maintain accurate documentation of support requests within the ticketing system. Manage incoming and outgoing faxes from TEH providers, ensuring timely routing of clinical forms and coordination of required signatures. Support care coordination initiatives and projects as directed by the Director of Care Coordination. Additional duties as assigned. Qualifications: 2-5 Years of experience working in a post-acute setting strongly preferred. Experience as a Medical Assistant and a MA certification required. Ability to work cross-functionally with clinical, operational, and administrative teams. Excellent written and verbal communication skills. Strong knowledge of Microsoft Suite products and systems. Skilled in data and order entry. Physical Requirements: Ability to perform the essential job functions consistently and successfully with the ADA, FMLA and other federal, state, and local standards, including meeting productivity standards. The employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel objects, tools, or controls; reach with hands and arms; climb stairs; balance; stoop; kneel, crouch or crawl; talk or hear. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Prolonged periods of sitting at a desk and working on a computer, tablet or mobile device. Must be able to lift up to 15 pounds at times. Working Conditions: Work will be performed in a home office setting. Must maintain a HIPAA compliant dedicated space in the home office setting to complete documentation and to conduct telehealth visits. This space must provide privacy to protect PHI/PII and provide a professional backdrop for video conferencing. Access to Wi-Fi required.
