The RN Clinical Liaison conducts clinical assessments, collaborates closely with physicians and the interdisciplinary hospice team, and documents accurately in EMR systems. The ideal candidate is a compassionate, mission-driven RN with strong communication, outreach, and relationship-building skills, committed to dignified end-of-life care.
