LCSW – Regional Intake Specialist Compensation: $80,000–$95,000 OTE/year Location: Regional Territory (Fort Myers, FL) — travel to 8–10 post‑acute/long‑term care facilities Job Type: Full-Time The Role We’re hiring a Licensed Clinical Social Worker to lead behavioral health intakes and care coordination across a regional network of 8–10 post‑acute and long‑term care facilities. You’ll conduct assessments, evaluate patients for behavioral health and Value‑Based Care programs, and support the clinical development of Behavioral Health Coordinators in the field. What You’ll Do Conduct psychiatric intakes and psychosocial assessments for diagnosisand treatment planning. Evaluate patients for eligibility across behavioral health and Value‑Based Care programs; obtain informed consent. Educate patients, families, and facility staff on available behavioral health services. Coordinate referrals, transitions of care, and handoffs across the care continuum. Serve as clinical escalation point for complex cases and conflict resolution. Maintain compliant EHR documentation and review Coordinator documentation for accuracy. Mentor and train Behavioral Health Coordinators, including onboarding new team members. Flag operational and staffing barriers to leadership. Support audits, quality reviews, and compliance initiatives. Travel across a defined region to support 8–10 facility sites. What You Bring Active LCSW license (or equivalent independent clinical licensure) in good standing. Experience with psychiatric intakes, psychosocial assessments, or behavioral health case management. Comfort with EHR systems and digital communication tools. Experience with post‑acute, long‑term care, or geriatric populations a plus. Reliable transportation and willingness to travel regionally.
