Speech Language Pathologist-PRN

Position Overview Become part of our caring community. As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you’ll play a vital role in helping patients regain strength, mobility and independence—all from the comfort of their homes. Responsibilities Evaluate, direct and provide speech/language pathology services to patients in the home or facility. Participate in the development and periodic review of the Plan of Treatment and Plan of Care. Use professional skills and judgment to assess and treat disorders of speech, voice, language, hearing and swallowing, preventing, identifying, evaluating and minimizing their effects. Administer and interpret diagnostic tests and therapeutic treatments, including audiology screening. Observe, record and report changes in the patient's condition and response to treatment to supervisor and/or physician. Provide instruction and training to patients in the use of alternative communication systems when appropriate. Provide counsel and instruction to patients, families and healthcare staff. Maintain and submit documentation as required by the company and/or facility. Prepare and submit written reports of evaluations, visits, summaries, care plans, care coordination activities and progress reports as required by company policy. Participate in care coordination activities and discharge planning. Maintain highest standards of professional conduct concerning confidential information, sharing only when appropriate and in the best interests of the patient. Required Qualifications Minimum six months experience as a speech therapist / speech-language pathologist. Current and unrestricted license. Current CPR certification. Valid driver’s license, auto insurance, and reliable transportation. Preferred Qualifications Home health experience. Compensation Pay per visit/unit: $60.00 - $83.00. Annual base pay range: $93,900 - $129,300. Additional Information Tuberculosis (TB) screening program: this role is patient‑facing and requires TB screening. Driver safety program: requires valid state driver’s license and maintenance of vehicle insurance at minimum limits. Scheduled weekly hours: 40 hours per week (full‑time).
