Facility: Acclaim Rehab and Nursing Center
Position: Home Health Aide (HHA)
Location: New York
Schedule: Part-Time | 7:00 AM – 3:00 PM
Pay Rate: $18–$20/hour

Acclaim Rehab and Nursing Center is seeking a dependable and compassionate Home Health Aide (HHA) to assist residents during clinic and dialysis appointments while ensuring their safety, comfort, and well-being.

Key Responsibilities

Escort residents to and from clinic and dialysis appointments
Arrive one hour prior to scheduled appointments
Assist residents in preparing for appointments
Ensure residents receive snacks and required documentation
Monitor residents to prevent falls or incidents
Report all incidents immediately to nursing leadership
Communicate with clinic staff and facility regarding resident status
Maintain professionalism and follow all facility policies

Qualifications

Current New York State HHA certification (required)
High school diploma or equivalent (preferred)
Strong communication skills and reliability
Nursing home experience (preferred)
