Functional Medicine Nurse Practitioner / Physician Assistant - Hybrid Role

Location: Louisville, CO
Job Type: Full-Time, Hybrid
Schedule: Monday-Thursday: 8:30 a.m.-3:30 p.m.
Friday: Remote administrative work

Job Summary:-

We are seeking a compassionate and motivated Nurse Practitioner (NP) or Physician Assistant (PA-C) with a strong interest in integrative and functional medicine. The ideal candidate provides personalized, comprehensive patient care and has completed or is actively pursuing formal functional medicine training or fellowship.

Responsibilities:-

Provide individualized, patient-centered care using functional and integrative medicine approaches.
Evaluate patients and develop personalized treatment plans.
Assess patient progress and adjust care plans as appropriate.
Maintain accurate clinical documentation and records.
Collaborate with patients and healthcare professionals to support optimal health outcomes.

Qualifications:-

Certified Nurse Practitioner (NP) or Physician Assistant (PA-C).
Current prescribing authority or eligibility to obtain the required prescribing privileges.
Experience in functional medicine is required.
IFM or A4M training or certification preferred.
Current DEA registration or eligibility to obtain.
Active Colorado NP or PA license.
