About the Position

The nurse will work in a day shift schedule, providing care management services. The candidate must have experience in acute hospital settings and demonstrate flexibility with scheduling. The role involves discharge planning and coordination of care, ensuring all regulatory guidelines are met. Candidates must pass a compliance EKG exam as part of the hiring process. Experience in case management and familiarity with CMS guidelines is essential. Strong communication skills and ability to work collaboratively in an interdisciplinary team are required. The candidate must be knowledgeable about HIPAA guidelines and maintain patient confidentiality. All applicants should possess a valid nursing license and relevant certifications.
