Occupational Therapist (OT) – Full-Time + PRN

Position Overview
The Occupational Therapist is responsible for evaluating residents, developing individualized treatment plans, and providing skilled therapeutic interventions that promote independence, safety, and quality of life. The OT works closely with residents, families, and the interdisciplinary team to achieve meaningful functional outcomes.

Key Responsibilities

• Evaluate patients and develop individualized treatment plans

• Provide skilled occupational therapy services to improve function and independence

• Collaborate with interdisciplinary team members to coordinate care

• Educate patients, families, and caregivers on treatment goals and home programs

• Monitor patient progress and modify treatment plans as needed

• Maintain accurate and timely documentation in accordance with state and federal regulations

Qualifications

• Skilled nursing facility (SNF) or long-term care experience preferred

• Knowledge of current occupational therapy practices and principles

• Experience with Net Health or similar EMR systems preferred

• Strong communication, organizational, and interpersonal skills

Licensure Requirements

• Current Occupational Therapist license in the State of New Mexico

• License must be active and in good standing
