Office-Based Addiction Treatment Nurse Primary Care

We are seeking a dedicated Office-Based Addiction Treatment (OBAT) Nurse to join our Primary Care team at Beth Israel Lahey Health. This unique role offers a blend of on-site and remote work, supporting patients across multiple locations. The OBAT Nurse will spend one day per week at Quincy Square Family Medicine, one day per week at Chelsea Internal and Family Medicine, with the remaining days working remotely. The OBAT Nurse position is primarily based in Chelsea and Quincy with plans to expand to additional locations. The clinic is designed for patients with substance use disorder and has a specific focus on equitable care, which seeks to eliminate disparities in access and retention by race, ethnicity, or language. The nurse is responsible for assuring competent, compassionate, individualized nursing care for patients with SUD. This role collaborates with all members of the care team to ensure coordinated, comprehensive, and seamless care transitions, provides education and outreach, and may require evening hours. The position reports directly to the Addiction Nurse Practitioner Program Manager and Director of Nursing. Responsibilities Assure competent, compassionate, individualized nursing care for patients with SUD. Collaborate with all members of the care team to coordinate comprehensive and seamless care transitions. Provide evidence‑based nursing care utilizing the nursing process and interdisciplinary teamwork. Actively manage patient flow, including rooming and triaging patients; expedite appropriate care and treatment using assessment, critical reasoning, and clinical judgment. Document nursing assessment, interventions, plans, and outcomes in the patient record in a timely manner. Assess, triage, provide nursing consultation, and perform follow‑up and surveillance of patient status and outcomes using telecommunications technologies. Create a therapeutic relationship with patient and family respecting cultural beliefs, values, individual needs, health goals, and treatment preferences; practice harm reduction. Provide health education to facilitate patient treatment, including medications for SUD and PrEP, safer drug use, and infection prevention. Work with outside pharmacy to obtain medication orders, prescriptions, refills, and communication as needed. Administer injectable SUD pharmacotherapy as indicated. Consult with members of the treatment team and other professionals to devise individualized psychosocial treatment plans. Participate in intensive case management within a team‑based, patient‑centered model of care. Review and follow‑up with patients and health care team to ensure timely completion of tests, screenings, and follow‑up care. Track and follow‑up on all critical referrals and test results; evaluate effectiveness of care and interventions. Attend all team rounds. Required Qualifications Graduate from an accredited Nursing Program (Bachelor’s degree in Nursing preferred). RN license required, or Licensed Practical Nurse license required. Basic Life Support (BLS) certificate required. 0–1 years related work experience. CPR certification. Understanding of harm‑reduction philosophy. Strong interpersonal, patient communication, and patient education skills. Advanced skills with Microsoft applications (Outlook, Word, Excel, PowerPoint, Access) and other web‑based applications; ability to produce complex documents, perform analysis, and maintain databases. Preferred Qualifications Experience in Addiction Medicine. Competencies Decision Making: Make decisions guided by general instructions and practices; recommend solutions for moderate‑complexity problems. Problem Solving: Address varied problems using observation, knowledge, and skills based on general precedents. Independence of Action: Follow precedents and procedures; set priorities; seek assistance when needed; supervisor/manager monitors progress. Written Communications: Communicate clearly and effectively in written English with internal and external customers. Oral Communications: Comprehend and converse in English to communicate effectively with staff, patients, families, and external customers. Knowledge: Demonstrate full working knowledge of standard concepts, practices, and policies; apply them in varied situations. Team Work: Work collaboratively in small teams to improve operations; offer ideas, identify issues, respect team members. Customer Service: Provide a high level of service to patients, visitors, staff, and external customers in a professional, respectful manner; remain calm in stressful situations. Age-Based Competencies Young adult: 16‑30 years. Middle age: 30‑60 years. Elderly: 60+ years. Physical Nature of the Job Medium work: Exert up to 50 pounds of force occasionally or up to 20 pounds frequently. Employee must stand and/or move around during the majority of their shift. FLSA Status Non‑Exempt. Vaccination Requirement All staff must be vaccinated against influenza (flu) and COVID‑19 as a condition of employment.
