About the Role:

As a CNA at Tucker House, you will be responsible for providing high-quality care to our residents, including:

• Assisting with personal care activities, such as bathing, dressing, and grooming
• Supporting resident mobility and transferring residents safely
• Assisting with mealtime and hydration
• Monitoring and reporting changes in resident condition
• Collaborating with the interdisciplinary care team to develop and implement individualized care plans
• Providing emotional support and companionship to residents and their families

Responsibilities:

• Provide direct patient care to residents, including vital signs, wound care, and medication assistance
• Document resident care and progress accurately and timely
• Participate in resident care conferences and contribute to the development of individualized care plans
• Maintain a safe and clean environment for residents, staff, and visitors
• Collaborate with other departments, such as rehab and activities, to ensure comprehensive care
• Provide emotional support and companionship to residents and their families

Requirements:

• Current certification as a Certified Nursing Assistant (CNA) in the state of Pennsylvania
• High school diploma or equivalent required
• Ability to work varying shifts, including days, nights, weekends, and holidays
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
