Roles & Responsibilities

• Prioritize and complete start of care visits for home health patients
• Supervise and support Physical Therapy Assistants to ensure high-quality care
• Complete clinical documentation within 48 hours of each visit
• Maintain timely communication with supervisors, intake, and office teams
• Coordinate care across a patient caseload in the Greater Sacramento area
• Be responsive and available during normal business hours to address patient needs

Qualifications

• Active and current California Physical Therapist license
• Minimum 1 year of Home Health experience
• Ability to supervise Physical Therapy Assistants
• Experience prioritizing start of care visits
• Must be responsive during business hours and meet documentation deadlines

Knowledge & Skills

• Strong clinical documentation skills, including Kinnser EMR
• Excellent communication and interpersonal abilities
• Supervision and delegation skills for therapy assistants
• Time management and prioritization in a home health setting
• Knowledge of start of care, recertification, evaluation, and discharge visit types
