Transitional Care Manager Drive Safe, Timely Patient Transitions

Home Recovery-Home Aid

Home Recovery Home Aid in Richmond, VA is seeking a compassionate Transitional Care Manager to coordinate safe transitions from acute and post-acute settings to home health services. You will work with hospital case managers, physicians, and referral sources to close care gaps and reduce readmissions.

Ideal candidates have an active RN license in Virginia and 2+ years in home health, case management, or discharge planning, with strong communication and EMR skills (WellSky).
