This critical field role involves direct patient care and comprehensive case management, aiming to enhance end-of-life care and improve patient outcomes. Salary: $82,500 - $97,800 Per Year, Depending on Experience Benefits: On-call stipends, Mileage reimbursement Type: Field role Hiring Decision: Made by client agency Responsibilities: Assess patient conditions and develop individualized care plans focusing on comfort and quality of life. Maintain detailed medical records and communicate patient status to relevant parties, ensuring seamless care coordination. Serve as the primary liaison among patients, their families, and healthcare providers, offering compassionate support and education. Educate patients and families on health management, palliative care options, and preventive measures to manage symptoms effectively. Coordinate with healthcare professionals to ensure continuous, patient-centered care and efficient case management. Conduct regular home visits to monitor patient progress and adjust care plans as necessary. Requirements: License: Active RN license in Arizona with no restrictions. Experience: At least 1 year of in-home hospice experience is required. Skills: Proficient in case management software, exceptional organizational and communication skills.
