LPN - LTCTryfacta is seeking an LPN - LTC for our client in Lee's Summit, MO 64081. This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please apply now!Position Title: LPN - LTCLocation: Lee's Summit, MO 64081Duration: 13-weekWork Schedule: DaysPay rate: $45- $55/hr.Responsibilities for this position include, but are not limited to:Provide direct nursing care to residents in a Long-Term Care (LTC) / Nursing Home setting.Administer medications and treatments as ordered.Perform and document resident assessments and monitor vital signs.Monitor residents for changes in condition and promptly report changes to physicians and the healthcare team.Perform wound care and dressing changes as needed.Assist residents with activities of daily living (ADLs) and provide compassionate, patient-centered care.Develop, implement, and update resident care plans as needed.Participate in resident care conferences and coordinate care with CNAs, nursing staff, physicians, and other healthcare professionals.Provide direction and supervision to CNAs and other nursing staff as assigned.Complete accurate and timely clinical documentation using PointClickCare (PCC).Follow infection control procedures, patient safety standards, and facility policies.Requirements:Active Missouri LPN License required.Minimum 1 year of Long-Term Care (LTC) experience required.BLS Certification required.Experience with PointClickCare (PCC) preferred/required based on facility workflow.Strong knowledge of medication administration, resident assessment, wound care, and care planning.Previous Charge Nurse experience preferred.Must be comfortable working Variable Day Shift: 12-hour shifts.Must be comfortable working rotating 36/48-hour weeks.Must be available to work every other weekend/Red Team schedule as assigned.OT is available, with additional hours based on facility needs and call-offs.Must be comfortable working in a LTC/Nursing Home environment.Tryfacta is an Equal Opportunity/Affirmative Action Employer.
