Heritage Senior Living is seeking a compassionate, organized, and motivated Assistant Director of Nursing (ADON) to help lead clinical services and resident care within one of our senior living communities.

Reporting directly to the Executive Director, the Assistant Director of Nursing is an important member of the community's clinical leadership team. The ADON partners closely with the DON to support resident care, clinical documentation, medication management, regulatory compliance, infection prevention, staffing, and the leadership and development of LPNs, CNAs, and caregivers.

Support Resident Care & Wellness

You'll work alongside the Director of Nursing to ensure residents receive individualized, compassionate, and high-quality care.

• Assist the DON with clinical operations and assigned responsibilities
• Conduct comprehensive resident assessments for prospective and new residents
• Assist with clinical admission and move-in requirements
• Develop and maintain Individual Service Plans (ISPs) within required timeframes and when resident needs change
• Conduct regular resident wellness check-ins and identify changes in condition or care needs
• Participate in resident care and ISP meetings with residents, families, providers, and appropriate team members
• Partner with residents and families to ensure medical and wellness needs are appropriately addressed
• Track physician appointments and communicate with providers as needed
• Accurately document physician orders and ensure changes are communicated to appropriate team members
• Ensure resident clinical, dietary, and other special needs are communicated to the care team before move-in

Oversee Medication Management & Documentation

You'll help maintain strong clinical systems that promote resident safety and regulatory compliance.

• Assist with oversight of the community's medication management program
• Monitor eMAR utilization and timely medication administration
• Conduct medication and clinical documentation audits
• Monitor medication schedules and help ensure an adequate supply of resident medications
• Receive and verify medication orders as appropriate
• Communicate medication and treatment changes to LPNs, CNAs, and caregivers
• Train and coach appropriate team members on medication administration policies and procedures
• Assist with periodic reviews of psychotropic medications with appropriate healthcare providers
• Maintain timely and accurate Point of Care (POC) documentation
• Monitor PointClickCare alerts and dashboards and ensure appropriate follow-up
• Identify documentation or medication-management concerns and work with the DON to implement corrective action

Lead & Develop the Care Team

Great resident care starts with a knowledgeable, confident, and supported care team.

• Provide day-to-day clinical leadership and support to LPNs, CNAs, and caregivers
• Assist the DON with supervision, coaching, and development of the care team
• Support appropriate oversight of LPNs serving in management roles
• Assess employee competencies and identify opportunities for additional education and development
• Participate in recruiting, interviewing, onboarding, and training clinical employees
• Provide education related to resident care, medication administration, documentation, infection prevention, and clinical procedures
• Assist with employee performance management and accountability
• Partner with the DON and Executive Director to develop and maintain appropriate staffing schedules
• Help create a collaborative, respectful, and resident-centered culture

Drive Clinical Quality & Compliance

You'll partner with the DON to ensure the community maintains high standards of quality, safety, and regulatory compliance.

• Develop and maintain knowledge of applicable federal, Wisconsin, and company regulations, policies, and procedures
• Assist with maintaining clinical and regulatory compliance
• Review daily clinical logs, documentation, and resident concerns
• Monitor infections, accidents, incidents, and other clinical quality indicators
• Ensure infection prevention and control procedures are consistently followed
• Assist with incident investigation, documentation, and appropriate follow-up
• Support applicable OSHA and workplace safety requirements
• Participate in clinical audits and corrective action plans
• Help ensure the community is prepared for regulatory inspections and surveys
• Identify clinical risks and promptly escalate significant concerns to the DON and appropriate leadership

Coordinate Resident Care

Strong communication and coordination are essential to delivering exceptional resident care.

• Participate in and help lead daily Care Focus meetings with the DON
• Participate in and help lead weekly care coordination meetings
• Monitor resident changes in condition and ensure appropriate follow-up
• Communicate resident needs and changes to appropriate employees, families, and healthcare providers
• Collaborate with physicians, pharmacies, hospitals, hospice, home health, therapy providers, and other healthcare partners
• Help residents and families navigate changes in care needs
• Serve as an accessible clinical resource for residents, families, and employees

Support Community Operations

As a member of the community leadership team, you'll also contribute beyond clinical operations.

• Work collaboratively with the Executive Director, DON, and other department leaders
• Assist with prospective resident inquiries, assessments, and community tours as needed
• Support staffing and scheduling based on resident and community needs
• Participate in community leadership meetings and initiatives
• Promote Heritage Senior Living standards and a strong customer-service culture

Qualifications

• Graduate of an accredited school of nursing
• Current and valid Wisconsin nursing license in good standing
• At least one year of nursing experience working with older adults preferred
• Assisted living, memory care, skilled nursing, or other long-term care experience preferred
• Previous supervisory or nursing leadership experience preferred
• SNF and/or assisted living experience preferred
• Dementia and memory care experience preferred
• Experience supervising, training, or developing LPNs, CNAs, and caregivers is a plus
• Excellent written and verbal communication skills
• Proficiency with Microsoft Office applications
• PointClickCare experience is a plus
• Ability to meet applicable pre-employment and ongoing requirements established by state and federal regulations
